Self-referral

If you are worried about your drug or alcohol use you can refer yourself to our service.

Getting in touch with us directly helps us to get to know you and understand your needs better from the beginning.

If you're worried about someone - a family member, client, patient or friend - you can refer them to i-access to get support with their consent.

Taking the first step is simple:

We will write to your GP to tell them about your care and any medication we prescribe, unless you ask us not to.

If you would like to visit us in person, please call 0300 222 5932 and we will give you the details of your nearest local service.

In the meantime, find out more about staying safe.

Please be aware that we are not an emergency service.

Referrals from professionals

If you are a GP, social worker, mental health professional, work in probation, or work for a voluntary substance misuse provider or in a hospital please use this referral form. Please ensure you have consent from the person you are referring.

If you would like to upload our referral form to your EPR please use this unrestricted referral form.

What happens next

When we receive your information, we review all your details and consider the next steps and what will work best for you. We'll get in touch with you within 7 working days.

If our service is right for you, we’ll offer you an appointment for an assessment. This may be by telephone or in person at one of our clinics in Surrey - we do our best to arrange appointments at your nearest clinic location. Once this has taken place we will develop a personalised recovery care plan with you.

If our service isn’t right for you, we will discuss more suitable services that we think will be able to help you.
